E-forms (both G & S) and related materials are available at SFAA's
website starting 1 April (Sun):-

Appointment and Collection of Completed E-Form
===============================================
Step 1
------
Make an appointment (from 18 Apr (Wed) to 24 Apr (Tue)): at the Student
Affairs Office (5/F, via lift 3) during office hour within the period.
Appointment tags are distributed on first-come-first-served basis. No
appointment can be made after 24 April.
Step 2
------
Submission of forms (from 25 Apr (Wed) to 3 May (Thu)): completed
application form together with copies of supporting documents should be
submitted IN PERSON according to the time of appointment at Room 4006
Workshop (4/F via lift 4) within the period.
Form collection period: 25 April to 3 May 2007 (10am to 5 pm daily from
Mon to Fri)
Application deadline: 3 May 2007 (Thursday)

IMPORTANT NOTE ABOUT INCOMPLETE INFORMATION
===========================================
Make sure to give full and accurate information and documents as
required. INCOMPLETE APPLICATION will be processed the LAST, resulting
in LATE payment. APPLICATION FORMS WITHOUT 2D BARCODE PAGE WILL NOT BE
ACCEPTED.
Therefore, you are strongly advised to read the Guidance Notes
(available on-line ONLY) carefully before completing the E-form and
double check the information contained in and documents enclosed with
the form in order to avoid any omission of information. Make good use of
Table 7 of the form to report any document(s) which is/are not yet
available.
Students currently on exchange
==============================
Once you return to HK, please submit the completed e-forms together with
all the supporting documents to us ASAP. We will send the completed
forms to the SFAA as soon as the forms are received. An explanation
letter detailing the reason(s) of the late application should be
submitted at the same time. |
